Serum ghrelin and other hormones in obese and overweight children and adolescents

Ghrelin hormone is known to play an important role in glucose metabolism and in cellular differentiation and proliferation through growth hormone (GH) like action.

Objectives: To study the serum ghrelin level in overweight and obese children and adolescents, an additional aim was to study the relationship between serum levels of ghrelin hormone and levels of GH, cortisol, ACTH and glucose. Patients were divided into 2 groups according to BMI; Group I consisted of 40 overweight children and adolescence while group II consisted of 40 obese and children adolescent. Forty healthy age and sex matched subjects were selected as control group. All groups were subjected to history taking, clinical examination and laboratory investigations including measurement of serum ghrelin level, GH, cortisol, ACTH and glucose (fasting and postprandial).

Results : Mean serum ghrelin was lowest among obese and highest among the control groups (p< 0.001). Serum ghrelin correlated negatively with BMI in the obese (group II) only and it was associated with low GH and high cortisol levels. In overweight and/or obese children and adolescents, the correlation of serum ghrelin with the blood glucose and ACTH levels was not significant (P>0.05). A negative correlation was found between serum ghrelin and BMI in obese group and it was associated with low GH and high cortisol levels.

Conclusion: Overweight and obese children and adolescents were characterized with low serum ghrelin level . Accurate and appropriate assessment of overweight and obesity in children and adolescents was recommended and further studies should be done on the important role of ghrelin in managing the complications
