Objectives: In this study, to analyze the harmful effects of nicotine and beneficial effects of resveratrol and cilostazol on flap viability of rats exposed to nicotine, the oxidant and antioxidant parameters, DNA turnover enzyme activities were examined.

Methods: Rats were divided into four groups: Control (Group 1), Nicotin (Group 2), Nicotine and Resveratrol (Group 3), Nicotine and Cilostazol (Group 4). Subcutaneous nicotine injections of 2 mg/kg/day were administered to rats in group 2, 3, and 4 for two weeks preoperatively. Group 3 rats received 40 mg/kg/day resveratrol orogastricly for 7 days beginning with surgery while Group 4 rats received 100 mg/kg/day cilostazol like group 3 treatment procedure. A dorsal peninsula flap (McFarlane flap) was planned on the rat dorsum. Postoperative 7th day full thickness biopsies were taken from necrotic borders for analyses. Malondialdehyde (MDA) levels, superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GSH-Px), xsantine oxidase (XO), and adenosine deaminase (ADA) enzyme activities were analyzed in the flap tissue.

Results: ADA and XO enzyme activities showed a statistically significant increase in the groups 3 (p<0.05) and 4 (p<0.01) (resveratrol and cilostazol administered groups) compared to control group. In group 3, MDA levels were found the lowest within the groups although this was not statically significant. Also for GSH-Px, CAT, SOD there were no differences between the groups

Conclusion: According to decrease in MDA levels in resveratrol administered group, we suggested that resveratrol played an antioxidant role on the flap viability. Since ADA and XO are DNA turnover enzymes, statistically significant increased ADA and XO enzyme activities that we found might be due to increased cell generation and DNA synthesis. These findings explain the beneficial effect of resveratrol on wound healing.
